前立腺肥大(BPH) 15670名のと、非BPH 15670名
65歳以上が主で、平均年齢 67歳
両コホートとも主たる合併症は高血圧 (64.0%)
フォローアップ平均期間 4.83 ± 2.23 年間、 4.52 ± 2.29 年間
図1は、フォローアップ期間中、累積心房細動(AF)発生比率は非BPHに比べBPHコホート有意に高い (log‐rank test, p = 0.003)
全体の発生密度は1千人年あたり 4.77 (95% CI = 4.56‐4.99)と3.76 (95% CI = 3.59‐3.93)
年齢、高血圧合併、糖尿病、高脂血症、CHD、心不全、COPD、PAOD、慢性腎臓病、甲状腺機能亢進症、睡眠障害、痛風補正後、BPHコホートは非BPHに比べ有意にリスク高い
(補正HR = 1.19, 95% CI = 1.11‐1.28)
結論:BPHはAF発生率増加と関連

Mirabegron is a potent agonist of the human β3 adrenoceptor. However, in vitro and in vivo studies have also shown effects on β1 adrenoceptors [39,40].一部抜粋・・・
Oral administration of mirabegron has been seen to increase HR in dogs and humans [41,42]. This effect is dose-dependent, and much more evident at supra-therapeutic doses [43].
Likewise, mirabegron could increase systolic blood pressure by increasing β1 inotropic effects. However, in one study on dogs, mirabegron was associated with a decrease in SBP; this could be partially due to the presence of β3 receptors in the vascular system in dogs, which could explain a vasodilatory effect. Alternatively, it could be the result of β3 negative inotropic effects. However, this mechanism is not clear and further studies are needed [46].
完全にβ1作用がないわけではなさそうだ・・・
