iPhoneのApple app storeや AndroidなどのGoogle Playで「Symptom checker"や"medical diagnosis"と検索されたアプリの検証

45の標準化された患者ビグネットをコンパイルし、緊急性トリアージのカテゴリー3区分としたもので、例えば肺塞栓のような緊急ケア必要なもの、中耳炎のような非緊急性なもの、ウィルス性上気道感染のような自己管理可能なものに分ける。

診断を提供する徴候チェッカーによる主要アウトカムが初期20の候補診断を正確に当てられるか?本来の意味から言えば正確に緊急状態を篩い分けられるか?

米国FDA:NSAIDs使用と心発作・卒中リスク増加に対してさらなる警告

今のところは、選択的COX2阻害剤や非選択的NSAIDs区別無く警告

Prescription NSAID labels will be revised to reflect the following information:


  • The risk of heart attack or stroke can occur as early as the first weeks of using an NSAID. The risk may increase with longer use of the NSAID.:NSAID使用初回週など早期に心臓発作・卒中リスクが生じる。リスクはNSAID長期使用にて増加。

  • It was previously thought that all NSAIDs may have a similar risk.   • NSAIDs can increase the risk of heart attack or stroke in patients with or without heart disease or risk factors for heart disease. A large number of studies support this finding, with varying estimates of how much the risk is increased, depending on the drugs and the doses studied.:NSAIDsは心発作既往有無・心疾患リスク要素有無に関わらず、心発作・卒中リスクを増加させる。この知見に関しては多くの研究が有り、リスク増加がどの程度か様々な推定があり、それは薬剤や投与量に依存した推定である。
  • In general, patients with heart disease or risk factors for it have a greater likelihood of heart attack or stroke following NSAID use than patients without these risk factors because they have a higher risk at baseline.:一般に心疾患患者・心疾患リスク要素揺する患者では、ベースラインリスク要素それらリスクがない患者よりNSAID使用後の心発作・卒中尤度増加する。
  • Patients treated with NSAIDs following a first heart attack were more likely to die in the first year after the heart attack compared to patients who were not treated with NSAIDs after their first heart attack.:心発作後NSAIDs治療患者はNSAIDs治療のない患者に比べ初年度死亡率増加する。

リス媒介致死的脳症:VSBV-1

2年間で、ドイツのリスのブリーダー3名 ボルナウィルスによる致死的脳炎となった

年齢が63歳、62歳、73歳で、発熱、振戦、精神運動性低下、混乱、歩行困難、ミオクローヌス、眼球麻痺などを生じた


variegated squirrel 1 bornavirus (VSBV-1) が原因で、カワリリス(Sciurus variegatoides)が媒介する人畜共通感染症と判明



髄液細胞増加、MRIにて脳皮質・基底核・髄膜病変認めたが、生存中病原体同定できず。組織浮腫・壊死、グリア活動性・リンパ球浸潤あり



Mammalian 1 bornavirusと類似した5つのRNA配列切片metagenomic sequencingで、肝臓、腎臓、胸腔液から検出。PCRにてBeerらはリスのサンプルや3名の患者全員の盗血脳切片から同定し原因を解明した

全体のサンプルは、喫煙 14,555、非喫煙 273,162
精神疾患初回エピソードを有する喫煙頻度は 0.57 (95% CI 0.52–0.62; p<0 .0001="" blockquote="">

症例対照研究では、精神疾患初回エピソード包括オッズ比(喫煙 vs 非喫煙)は、 3.22 (95% CI 1.63–6.33)、 出版バイアスの存在有り  (Egger's test p=0.018, Begg's test p=0.007)


前向き研究にて、新規精神疾患包括的相対リスク(連日喫煙 vs 非喫煙) 2.18 (95% CI 1.23–3.85)

連日喫煙の場合、非喫煙と比べ、精神疾患若年発症 (比重差平均 −1.04 years, 95% CI −1.82 to −0.26)

精神疾患発症後喫煙が健康対照と比べ若年発症というわけではない (−0.44 歳 , 95% CI −1.21 to 0.34)

REGARDS研究: 卒中後認知機能低下

認知機能低下は卒中後遺障害の主要原因であるが、卒中後認知機能低下の時間推移は判明してなかった。卒中後6年間で、その機能は加速し、持続することがわかった。

卒中は全般認知機能、新規学習、言語記憶急激悪化と関連 (0.10 ポイント[95% CI, 0.04 to 0.17]、1.80 ポイント [95% CI, 0.73 to 2.86]、0.60 ポイント [95% CI, 0.13 to 1.07]).

卒中患者は、卒中無しのヒトと比べ、全般認知機能急激低下 (0.06 ポイント /年 より急激 [95% CI, 0.03 to 0.08]) 、遂行機能の急激低下と関連(0.63 ポイント /年より急激 [95% CI, 0.12 to 1.15]),
しかし、新規学習、言語記憶は関連せず

卒中生存者において、卒中後の認知機能障害リスクの差は、直後に比べ統計学的差認めず (odds ratio, 1.32 [95% CI, 0.95 to 1.83]; P = .10);

しかし、卒中前に比べ、認知機能障害発生は卒中後急激悪化する (odds ratio, 1.23 per year [95% CI, 1.10 to 1.38]; P < .001).


ベースラインでの全共役指標に対する平均値である70歳黒人女性では、卒中3年後、認知機能低下頻度は絶対的差は、4.0% (95% CI, −1.2% to 9.2%) 、 12年次 12.4% (95% CI, 7.7% to 17.1%)

1秒量Trajectory:若年緩徐悪化型vs高年急激悪化型 前者の方が圧倒的に多く、受動喫煙などの影響も考えられる

COPDの1秒量の年経過の足跡を検討すると
・若年40歳未満から徐々に悪化するタイプ
・高年から急激に悪化するタイプ
2つのタイプがある。前者のグループが全COPD85%ほどで多数。


さらに言えば、思春期すでに1秒量低下している一群もいるかもしれない。
受動喫煙の深刻な健康被害を示唆

【背景】COPDは1秒量の経年的減少加速をもたらすとされる。
しかし、一般正常未満だが、最1秒量最大現のCOPD患者で一秒量減少が正常で有ることも考えられる。
【方法】
We stratified participants in three independent cohort
3つの独立コホート被験者 (Framingham Offspring Cohort,  Copenhagen City Heart Study,  Lovelace Smokers Cohort) 、コホート登録時(平均年齢、ほぼ40歳)肺機能(1病量予測比 80%以上、80%未満)、と調査最終日COPDの存在有無。
経時的1秒量減少率をコホート開始時と、終了時COPD状態に従い検討



【結果】
40歳前、1秒量予測比 80%未満657名のうち、観察22年後COPD 174名(26%)
40歳前、1秒量予測比80%以上2207名のうち、観察22年後COPD 158(7%) p < 0.001

同様の喫煙暴露に関わらず、
観察終了時COPDの332名の約半数は、40歳前では1秒量正常、その後1秒量急激減少し、年間 53±21 ml/年
残りの半数は、成人早期より1秒量低下し、徐々に1秒量減少 27±18 ml/年 p < 0.001



【結論】
今回の研究では、成人初期から1秒量低下の場合はCOPD発症の端緒として重要。1秒量減少加速的減少は、COPDの必須というわけではない (Funded by an unrestricted grant from GlaxoSmithKline and others.)



Fletcher原図ってどこからわき出したのだろう・・・1960年頃大規模な経年的調査されたとはにわかに信じがたいのだが・・・

無批判にこの図譜を引用しまくっているが・・・



Cecilなどには、それを改変している・・・ 引用論文記載せずに・・・天下の内科学教科書なのに・・・

テトリスで遊ぶと、外傷後ストレス障害の不意に現れる不快な記憶去来数減少

外傷性イベントの記憶は数時間で固定化し、眼を閉じたらお呼びでない記憶、 intrusive memoryが繰り返しフラッシュし、心理消耗させる。
認知タスクを用いて記憶可塑性の段階で、reconsolidationをブロックできないかという検討。


認知タスクに、テトリスを用いる方法

実験的トラウマ受傷後、テトリスをプレイし、24時間メモリー再活性化タスクを行う
